Navigating the National Disability Insurance Scheme (NDIS) can be overwhelming. At AIVA Healthcare, our Support Coordination service is designed to assist participants in understanding their NDIS plan, coordinating services, and making the most of their funding to achieve meaningful outcomes.

Our Support Coordinators are experienced professionals who provide tailored assistance based on each individual’s goals, preferences, and level of need. We work collaboratively with participants, families, service providers, and stakeholders to ensure a cohesive, effective, and person-centred approach to care.

What is Support Coordination?

Support Coordination is a Capacity Building support within the NDIS that helps participants:

  • Understand their NDIS plan
  • Connect with a mix of informal, mainstream, and funded supports
  • Coordinate services across multiple providers
  • Resolve service delivery issues or challenges
  • Build the skills needed to independently manage their plan in the future

At AIVA Healthcare, we offer two levels of Support Coordination:

Support Coordination (Level 2)

General assistance for participants who need support navigating and coordinating their plan. This includes service sourcing, scheduling, and problem-solving.

Specialist Support Coordination (Level 3)

For participants with more complex needs or high-risk environments, requiring a higher level of coordination, advocacy, and interagency collaboration.

Understanding Your NDIS Plan

  • Breaking down the plan into clear, practical components
  • Identifying available supports under each budget category
  • Assisting with plan implementation timelines

Service Connection and Coordination

  • Sourcing appropriate providers based on your preferences and goals
  • Coordinating appointments and service delivery across multiple providers
  • Supporting agreements and service bookings in the NDIS portal

Monitoring and Reporting

  • Monitoring the effectiveness of current supports
  • Identifying service gaps or underutilisation of funding
  • Preparing progress reports and documentation for plan reviews

Capacity Building and Skill Development

  • Building participant confidence to self-manage or direct services
  • Supporting decision-making and problem-solving skills
  • Educating participants on rights, responsibilities, and advocacy

Crisis Support and Specialist Coordination

  • Assisting with complex needs, housing instability, or mental health-related risks
  • Coordinating support teams and liaising with health, justice, and community services
  • Advocating on behalf of the participant in critical situations

Who Is Eligible for Support Coordination?

Support Coordination is funded in a participant’s NDIS plan under Capacity Building – Support Coordination. Not all plans include this funding by default, but participants with multiple supports, complex needs, or limited informal networks are typically eligible.
