Community nursing plays a critical role in maintaining health, preventing complications, and supporting independent living. We focus on continuity of care and clinical excellence through proactive assessments, treatment planning, and close collaboration with participants, families, GPs, and other healthcare professionals.

Our Community Nursing Services Include:

Comprehensive Health Assessments

Initial and ongoing assessments to evaluate the participant’s overall health status, identify risks, and determine nursing care needs.

Individualised Care & Treatment Planning

Development and implementation of personalised care plans in consultation with participants and their support network, aligned with NDIS goals.

Wound Management

Clinical management of acute and chronic wounds, pressure injuries, surgical sites, and ulcers using current wound care protocols and dressing techniques.

Medication Management

Administration of prescribed medications, including injections and infusions, as well as monitoring for side effects and ensuring medication compliance

Chronic Condition Support

Ongoing nursing care for individuals with conditions such as diabetes, respiratory illness, cardiovascular disease, or neurological disorders

Complex Bowel Care

Specialist nursing interventions to manage complex bowel conditions, including enemas, manual evacuation, and monitoring of bowel function.

Continence Management

Support with continence assessments, catheter care, stoma care, and bladder and bowel health strategies to promote dignity and comfort.

Post-Hospital Discharge Support

Transition care for participants recently discharged from hospital, including wound checks, medication administration, and general monitoring to reduce the risk of readmission.

Personal Care with Clinical Oversight

Support with hygiene, dressing, grooming, and mobility tasks where clinical supervision is needed due to medical complexity.

Other Registered Nursing Services

Our RNs can also assist with PEG feeding, diabetes management (including insulin administration), palliative care, and coordination with medical specialists and allied health professionals.
